CDS has served dentistry since 1864. Associate Membership is how dentists who practice outside our Cook, Lake, and DuPage County boundaries stay connected to Chicago dentistry, from Midwinter to monthly CE.
Eligibility
- You are a dentist who practices outside Cook, Lake, and DuPage counties, including outside the United States.
- U.S. dentists: must be a member in good standing of the American Dental Association and your state and local dental societies.
- International dentists: ADA membership isn't required. You should belong to the primary dental organization in your country.

How much does Associate Membership cost?
Associate Membership is $125 per year, payable in U.S. funds.
I practice in Chicago. Can I join as an Associate?
Associate Membership is for dentists who practice outside Cook, Lake, and DuPage counties. If you practice inside those counties, you join CDS through tripartite membership with the ADA and the Illinois State Dental Society. See the dues structure for details.
I practice outside the United States. Am I eligible?
Yes. International dentists are welcome. ADA membership is not required, but you should belong to the primary dental organization in your country.
Can Associate Members vote or serve on the Board?
Associate Members do not vote on Bylaws changes, hold committee voting rights, or serve on the Board of Directors. These roles are reserved for CDS Members. Associate Members can volunteer on Midwinter Meeting committees.
What do I need to apply?
U.S. applicants will need their ADA number and proof of current ADA membership. Complete the online form and submit your $125 dues to finish joining.
