Official Letter of Invitation

The Chicago Dental Society will send an official letter of invitation to attend the Midwinter Meeting only upon completion of the Visa Letter Request Form below. This invitation is intended to facilitate your travel and visa arrangements but does not extend financial support. You must register for the Midwinter Meeting prior to completing the form.

Visa Requirements

It now takes as long as four months to obtain a Visa to enter the United States. Please use the following links for more information regarding obtaining visas, machine readable passports and registering with the U.S. Homeland Security’s ESTA program.

International attendees who require a letter of invitation from the Chicago Dental Society may request to receive a visa letter inviting them to enter the United States in order to participate in the Midwinter Meeting. U.S. citizens do not need to submit a request for a visa letter.
