Request a Donation of Dental Supplies to Support Your Event
- If you are a CDS member dentist in Cook, Lake or DuPage County, you may apply for a donation online.
- If you are a school, child care, or nonprofit organization serving children in Cook, Lake and DuPage Counties, the deadline has now passed to request supplies for 2012. You may contact Rachel Azark to request to have your school put on the mailing list to request supplies for February 2013.
Please note: Requests are first-come, first-served. Sorry, but because of the volume of calls we receive about this promotion, we can not accept phone requests.
