When a Patient Has a Complaint about a Dentist
CDS offers mediation services for patients and dentists to help resolve complaints about dental care without the expense of a court case.
What should the patient do?
The first step is for a patient to complete the Request for Review of Dental Services form. Working with Illinois State Dental Society (ISDS), CDS will then assign the request to members of the CDS Mediation and Peer Review Committee for their review. Committee members may ask to interview or examine the patient and talk with the dentist who provided treatment.
The entire process is free and strictly confidential. Dentists choose whether or not to participate. In most cases, CDS is able to resolve complaints to both parties' satisfaction.
Because ISDS oversees the program, patients must complete the Request for Review of Dental Services form and mail it to:
